ATTENDEES' REGISTRATION (ONLINE & IN SITE)

INSTRUCTIONS

You may use this form to pre-register for the Santiago conference up until September 15th, 2008 SEPTEMBER 25th, 2008 midnight! After that time, you can register at the door at the Universidad Cardenal Silva Henríquez.

  • ALL FIELDS MUST BE COMPLETED. INCOMPLETE FORMS WILL NOT BE PROCESSED. Please avoid using Spanish characters, except for the field " Name (as it must appear on your certificate)";
  • NOTE: Annual memberships in TESOL Chile are for 12 months, running from October 1st to the following September 30th;
  • No surcharge for "at the door" payments.

For your pre-registration to be completed, you must submit this form AND make your payment by the correct payment deadline SEPTEMBER 25th, 2008

PLEASE forward your confirmation notice or other proof of payment to TESOL Chile by e-mail at conference@tesolchile.cl. We will acknowledge your payment by email as well.

If you join TESOL Chile and/or pre-register for the conference online, you need to pay by electronic bank transfer from a Chilean bank. The following information is necessary for the transfer:

Important Notes

If you are registering for the conference or joining TESOL Chile as a student, you must have a valid current student ID card from your school. You should submit a clear copy of your Student ID to TESOL Chile by e-mail; you will still need to show your student ID when attending the conference.

If you do not live in Chile, please write to conference@tesolchile.cl for additional payment instructions.

Diplomas will be sent by email after conference is over.

IF YOU DO NOT SEE THE SECURITY IMAGE AT THE END OF THIS FORM, MAKE A PRINT OUT. YOU WILL SEE IT ON PAPER. TYPE IT IN AND SUBMIT.
